1. G.Abuladze, I. Jashi, D. Bakhtadze, E. Narsia, N, Chanturia Institute of Cardiology, Tbilisi. Diagnostic and Therapeutic Aspects of Atrial Fibrillation in the Light of New Guidelines                       

 Summary. The paper is dedicated to therapeutic and preventive efficacy of antiarrythmic drugs and    to the new classification of AF worked out in 2001 by American College of Cardiology, American Heart Association and European Society of Cardiology.Based on the study performed in the Institute on 92 patients with AF, the article provides description of the effects of sotalol, propafenon and amiodarone in prevention of AF recurrence. The results of the study establish high efficacy if the combination of amiodarone and ACE inhibitor enalapril, which proved to have stronger antiarrythmic action and was found to reduce left atrium dimensions in patients with AF secondary to organic heart disease.
 


2.   P. Aladashvili, N. Gegeshidze, V. Sharabidze. National center of tuberculosis and lung diseases. Tbilisi. Usage of extra corporal membrane oxygenation, hemabsorbtion and other extra corporal detoxic methods in treatment of patients infected with tuberculosis.
Summary.
The article deals with the usage of extra corporal membrane oxygenation, hemabsorbtion and other extra corporal detoxic methods in treatment of patients infected with tuberculosis. As a result of the research the following conclusions were made:

        The above-mentioned method is a procedure which causes fewer traumas. It may be used with different forms of tuberculosis.
 

·        EMO+hemabsorbtion, lasotherapy and ultraviolet ray of blood improves both blood reology and perfusion of the bodies, and thus minimizes the possibility of recidiv development.

·        Increase of immunity and blood elements, especially T and B lymphocyte level, favors decrease of quantity of medicines as well as durance of treatment.
 


3. Alibegashvili M., Kitiashvili I., Managadze L., Chigogidze T., Kotrikadze N.Iv. Javakhishvili Tbilisi State University, Faculty of Biology, Faculty of Medicine. Investigation of lipid spectrum of blood erythrocyte membranes in patients with prostate tumors.

Summary. Lipid content of erythrocyte membrane, especially phospholipids, cholesterol and lipid peroxidation intensity in patients with prostate benign hyperplasia and prostate adenocarcinoma has been investigated.
In addition to the increase of total amount of phospholipids, corresponding rise of cholincontaining phospholipids portions and decrease of aminocontaining phospholipids has been observed.
In both pathologies the increase of cholesterol quantity and intensity of lipid peroxidation has been established. Obtained data must be contributed to the systemic character of tumors.

Abstract. The patient with ovarian histoma  and uterus cista showed the highest  titre of anty-hCG autoantibodies out of the patients studied.The patient with  malignant tumours hed non-appreciable levels of anti-hCG antibodies.
However a correct measurement of the serum hCG β is technically complex which limits its application for tumour screening. the serum expression of hCG in malignancies may lead  to the induction of specific autoantipodies,which can represent a beeter serum marker.In addition it  will demonstrate a possibility of a recognition of the tumor- associated antigens.

Andriashvili, Therapy House. LTD. Tbilisi. Usage of the principles of anthroposophical Medicine, in treafment of Bronchial Asthma.

Summary. The aim of presented study  is to  scientifically evaluate effectiveness of anthroposophic

    methods of asthma treatment, taking into account individual and recovery potential of the

    child based on salutogenetic theory.

       The research was performed by open controlled clinical observation of the target group.

   30 patients of the same sex, age and clinical form/stage of disease were studied.   

    Three groups were identified among them:

   1. Group 1 - classical methods of treatment

   2. Group 2 - classical and anthroposophic methods of treatment

   3. Group 3 - anthroposophic methods of treatment only
       We have  performed   the evaluation of effectiveness of the anthroposophic methods    of

    treatment on the basis of short-term and long-term (3-5 years) observation of the

    patients.

       Clinical presentation and  immunological status of pediatric patients with remission   of

    symptoms and exacerbated asthma were evaluated in all target groups.

       The  study will  be continued for two years in order to achieve the reliability of scien tifi- 

    cally valid  results.  Continuation  of  the  study  aims to include proposed antropho sopic

    method in treatment plan of pediatric patients.

 


6. Z. Antadze, M. Kukava.Chair of Neurology, Center for Medical-Biological Studies. Tbilisi. SEVERAL SPECIFICATIONS OF POSTTRAUMATIC STRESS DISORDERS
SUMMARY. The study purposed investigation of 110 war refugees from Abkhazia. In the most cases (70,9%) pronounced and prolonged PRSD had been revealed, which were conditioned by marked disfunction of psychosocial sphere. As a rule, PRSD was accompanied by sympathicotonia, insomnia and psychosomatic manifestations, expressed in a form of arterial hypertension and the chronic headache attacks.

Summary.   Instruction for using “Forest Gift. “Forest Gift” is a 50% sugar syrope of a neutral vegetative extract. It contains proteins, free aminoacids. vitamins, alcalydes, lipids, phitosterins, phenol mixtures, chlorophyle and other biologically active mixtures.
It is used in medisine as a radioprotector, for radiation theraphy of different kinds of tumours, treatment of vegetoneurosis and infections illnesses, and as a remedi for protection from irradiation.
“Forest Gift” is registered and certificated by the Ministry successfully used for the rediation theraphy in the Oncological Centre of Georgia.

V.Bakuradze, T.Chigogidze, V. Sulhanishvili. National Center of Urology, Tbilisi, Georgia. Changes of renal local blood flow in acute hemorrhage and sepsis
 Summary. We investigate the renal local blood flow (RLBF) in cortical (CZ), subcortical (SCZ) and medular (MZ) zones of the rabbits in acute sepsis and hemorrhage using H2 clearance method.
 In sepsis conditions we observed decreased RLBF in CZ and SCZ and relative increase in MZ. In case of acute hemorrhage and gram-positive sepsis RLBF was reduced in all zones. These changes were more severe in case of acute hemorrhage and gram-negative sepsis.     We confirm that in cases of acute hemorrhage or / and sepsis kidneys undergo deterioration of blood circulation in all zones.


 

9. K. Beltadze, M Kiziria, Al. Tsiskaridze, R Shakarishvili. Tbilisi State University, Faculty of Medicine, Department of Neurology and Neurosurgery. Diagnostic of acute leukoencephalopathies.

Summary. Recent advances in neuroimaging have markedly expanded the spectrum of diseases, known as rarities in the nearest past among them, to be considered by the clinician in the process of diagnostic workup. The presented case warns practicing neurologist about potential diagnostic collisions appearing in case of either underestimation, overestivation or proper correlation of clinico-anamnestic and neuroimaging data.
 



10. Bochorishvili, N. Bregvadze. State Medical Academy of Postgraduate Education, Tbilisi.  
Clinic case of cytomegalovirus encephalitis.

SUMMARY. Many epidemiological stadies have confirmed that severe congenital CMV disease resalts mainly from primary CMV infection in pregnant women.  We discass the case of intrauterine CMV infection in 9 month child born from supposedly primary infected mother. Hence it is recommended that women of childbearing age know their CMV serostatus. In women with confirmed primary infection amnionic fluid PCR can be used to assess the risk of CMV diease for fetus. In case of confirmed acute infection of fetus the abortion is only way out. Also it is recommended to diagnose CMV infection in newborn, by detection of spesific IgM antibodies and to administer immunoprophylactic

Summary. The article describes a clinical case that argues diagnostic criterions, appropriated to Dysthymic disorder in the historical attitude. The thoughtful differtial diagnostics in between dysthymia, cyclothymia, schizophrenia and personality disorder is conducted.

13. M.Burduli, G.Tabidze, M.Iakobashvili, N.Mamamtavrishvili, A.Gogishvili, E.Narsia. Tsinamdsgvrishvili Institute of Cardiology, Tbilisi, Georgia. Treatment Tactics for Patients wit  Type II Diabetees Melitus

55 patients with initially developed Diabetes Melitus have been examined. Lypid spectrum and glikemia were studied during 24 hours on the background of combined therapy with the use of micronized form of maninil/siopfor mix. Positive dinamic of the lipid spectrum was observed on the background of compensation of diabetes, which leads to a certain decrease of negative consequences of cardiovascular defects of patients with Diabetes Melitus Type II, Ishhemic heart disease, and arterial hypertension.
 


14. I. Burdzgla. Otoacoustic Emission and its Clinical Application

 Summary. The discovery of the phenomenon of otoacoustic emission (OAE) has dramatically changed the opinions on the functioning of the inner ear. The OAE is a sound of very low intencities which can be registered by means of a sensitive microphone in the outer auditory canal. Two main types of OAE have been identified – spontaneous and evoked. It has been established that the transient-evoked OAE ( TEOAE) is highly specific and characteristic for individual subjects. Moreover, it has been shown that TEOAE differs in the right and left ears of the same person. The conducted studies have been proved that TEOAE is a stable event. The TEOAEs of the same patterns were registered in our investigations when the intervals between separate tests were rather long – days, weeks or even months. The TEOAE is widely used in clinical audiology as the screening test of the hearing. Due to the peripheral origin, by TEOAE recordings it is possible to estimate the state of cochlea and to reveal the hearing pathology in newborns of the risk groups.
 


15. M. Chkhaidze. Chair of Urology at Medical Academy. Optimization of Hemostasis in TURP-operations by means of “Buta”
 Summary

It is described the new method for prevention of bleeding and TUR-syndrome by means of polymer substance “Buta”. The results of experimental (on 17 dogs and 10 rabbits) and clinical investigations (on 40 patients) show that the substance “Buta” preliminary injected into the prostate during the TURP operation undergoes a deep polymerization by the contact with the hot loop of resectoscope and creates a thin film on the resected surface of prostate, which embolises the small and middlesized blood vessels and prevents both the bleeding and absorption of irrigative fluid and TUR-syndrome. Peculiarities and advantages of the described method are discussed.

Summary
With the aim of increasing the effectiveness of a treatment process lasertherapy has been widely used in feto-maternal medicine. The most widespread and broadly applied type of laser radiation is a helium-neon laser (wave length-632,8 nm). Basic methods of lasertherapy. Treatment of pregnants by a laser beam is not in all cases of a monotherapeutic character and is applied in combination with medical preparations in minimum doses. At this stage the major objective is to exclude unfavourable collateral effects of mediactions on maternal organism, lessening of pharmacological load on fetal organism

19. Djaparidze N. Khardzeishvili O. Patologic Anatomy Department of the Georgian Medical University of Tbilisi. Feature of the distribution T and B lymphocytes in stroma of the endometrium with glandular hyperplasii, adenomatose and highly differenciated adenocarcinoma.
Summary.
The authors has studied the quantity and peculiarity of lymphocites in stroma of the endometrium with glandular hyperplasii, adenomatose and highly differenciated adenocarcinoma. The material consisted 26 uterine abrasio of the women with dysphunctional uterine bleeding.  Accoding  these investigasion with morphometrial studi of the character quantity and localization of the lymphocites. There  are differents among the quantity of the lymphocites by these  condicions. The lowest quantity of lymphocits is registered by glandutar hyperplasia, the highly quantity of lymphocits by highly differenciated adenocarcinoma.
In endometrium situated are presented solely by T lympocits, as to B lympocits they are absentin.
Number of CD-4 (T helper) and CD-8 (T suppressor) increase by highly differenciated adenocarcinoma.
The increase of T-lymphocytes with the growth of grandular anaplasii  and epithelium of mucous membrane of uterus in explained by a change in genetic specificity of ephiteliocytes and the emergence of their population with different antigene properties having lost the tolerance towards their own immune system
In the most probable that the antigene difference grows with the increase of anaplasii level, first manifasted by tissue atipia (glandular hyperplasii) and then turns into cellular atipia (adenomatose and highly differenciated adenocarcinoma).

THUS WIDE INTRODUCTION OF TUR ENABLES CONSIDERABLY CHANGE THE STRUCTURE OF RENDERING ASSISTANCE TO THE PATIENTS SICK BY URINARY BLADDER CANCER.

THIS METHOD HAS A DEFINITE ADVANTAGES IN COMPARISON WITH OPEN OPERATION ON URINARY BLADDER SLIGHT INJURY, GOOD VISUAL AND ULTRASONOGRAPHICAL CONTROL OF COMPLETENESS OF RESECTION, ABLASTIC CONDITION WHILE MANIPULATIONS IN ISOLATED CAVITY OF URINARY BLADDER.

 


21. N.A. Dvaladze, B.A. Mosidze, T.F. Bochoidze, N.Sh.Chkhetia. Eristavi National Center of Experimental and Clinical Surgery Immunocorrective photohemotherapy in surgical treatment of patients with lung cancer.
Analysis of the treatment of 350 patients with lung cancer is proposed in the study.
In virtue of wide immunologic monitoring of the immune status of the patients with lung cancer has been reserched.
Results of our study showed presence of immunodeficiency at the cellular link of immune system for patients with lung cancer. It increased togeather with progressing the disease and was characterized by rising the suppressive activity, lowering the ratio – Th/Ts, depressing the sensibilization of lymphocytes to the normal tissular (NA) and tumor antigens (TA), and decreasing the activity of the NK – and phagocyting cells.
Analysis of correlation between the immunologic status before operation and the effectiveness of surgical treatment, conducted by  a method of the grouping and the discriminative analisis, has revealed direct correlation between the LS – lymphosytes to NA and the survival of the patients with lung cancer. This analisis and initial low NK activily with association to a bad prognosis lead to necessity for using the immunocorrection.
Quantum hemotherapy in certain regimes of photomodification of blood have been found as an effective remedy for immunocorrection. It was displayed by reducing the cancentration of T-suppressors, rising the ratio Th/Ts, increasing the activity of the NK – and phagocyting cells. Taking the anti-inflammatory and antioxidantive action into account, the quantum hemotherapy should be considered as an effective remedy for preventing and treating the operative and post-operative complications, as well as a remedy for rising the resistance of the body of a patient with lung cancer to the radial and chemotheraputic treatments.

22. K. Ediberidze. A.Natishvili Institute of Experimental Morphology Georgian Academy of Sciences Tbilisi, Georgia. Ultrastructural peculiarities of human placenta in incomplete pregnancy of hormonal genesis.
Summary. Complex histologic and election microscopic  study of placenta is performed in 40 women with incomplete pregnancy (20-27 weeks). Inflammatory and dystrophic changes, also early immaturity of the stroma were described.
 


23. G. Galdava. Research Institute of skin and venereal diseases. Tbilisi. Some pecularities of secondary infertility in man.

Summary. From all the abovementioned we can conclude, that the development of secondary infertility in man is caused by variety of different factors. Research has revealed , that 45,6% of cased of secondary infertility are caused by sexually transmitted diseases and infectuas processes of urinaru and sexual organs; 12% of cases are caused by auto-immune processes that occur in organisms of man, and 3,2% of cases are triggered by unfriendly environment, and unsetisfactory working and living conditions. Timely identification of the above factors and adecuate treatment and preventive measures shall provide for substantial dicrease of overall rate of secondary infertility in man.

There are two ways of adaptation of human with environmental. The first - improvement of environmental preservation, the second - increase the stability of organisms towards the action of harmfull factors on the basis of the methods of bioorganic chemistry.
Adaptation of organisms to the different conditions is possible through the usage of bio-energo-activators, which were obtained in our laboratory.
Bio-energo-activator enables preservation of homeostasis through activation of physiological regulatory mechanisms, providing adaptation to the extremal conditions.
In our laboratory on the basis of bio-energo-activators many highly-effective medical preparations have been created. 25. K. GOGILASHVILI.,   E. JAPIASHVILI.,   L. ZHORZHOLIANI. QUONTITIVE AND QUOLITATIVE RISK FACTORS RANGING IN HYPOPLASIA DEVELOPMENT.

SUMMARY.        The present research aimed at integral study and assessment of  risk factors promoting spread and formation of hypoplasia among children. The survey covered 617 Georgian children at 6-16 years of age. Dental status was determined by standard scheme designed by WHO.  Data were processed according to SPSS/v.9 (statistical package for social sciences) program package. 
 Through the rating of the identified risk factors it was concluded that the most significant influence over the development of enamel hypoplasia are caused by unbalanced nutrition, excessive use of carbohydrates,  hereditary predisposition  to hypoplasia, artificial nutrition under one year age, mothers sickness during the pregnancy, accompanying somatic diseases and pathologic proceeding of the pregnancy
.

In  Batumi N1 hospital’s clinic of Neurology&neurosurgery  firstly in Georgia were used  ficsators and endoprotesis from  TiNi  with thermomechanical memory of form in case of vertebro-spinal injuries and diseases.

 


27. Gvakharia G. Nakaidze B. Gagua Z. Dandurishvili S.Iv. Javakhishvili State University  Surgical Department. The Diagnose And Treatment Of Diapragmh Post-Traumatic Hernia
 During the traumatic inguar of the abdomen and the chest hole, the diapragmh  destruction is a comparativeli rare patologhy. We represent  the three cases of diapragmh post-traumatic hernia, complicated with a stuck. The thoracoabdominal way of the operation was chosen. The stomach, spleen and a small intestine loop were stucked. It was done a gastrectomia, splenectomia, and in the third case-the stomach detorzio. All three operations were finished by the bringing down the organs in the abdomen, diapragmhoraphy and the drenning of the abdomen and chest. 

 


28. I. Jashi, B. Kobulia  M. Tsinamdzgvrishvili’s  Institute of Cardiology, Tbilisi, Georgia. Comparative Effects of Losartan and Enalapril on Renal Function and Hemodynamics in CHF
Summary
The aim of investigation was the evaluation of Enalapril (E) and Losartan (L) influence on electrolyte and water excreation, renal function and renal hemodynamics in pts with congestive heart failure (CHF) in accordance with  initial hemodynamic status.
83 male pts, aged 44 to 67, with CHF (II-III NYHA classification) due to chronic forms of coronary heart disease were examined. The pts were divided in 3 groups depending on the treatment: I group (29 pts) received cardiac glycosides and diuretics; E 10 mg bid was administered to 28 pts (II group) and L 12,5 mg once daily, titrated to 25 mg, and up to 50 mg once daily was administered – to 26 pts (III group). Acute pharmacological test followed by six-month course of E or L treatment was performed in each pts. Renal function was assessed by endogenous creatinine clearance, renal hemodynamics parameters – by cardiotrast clearance. Sodium and potassium ion concentration was determined by biochemical analyzer, sodium and water excretion was calculated by formula. Central hemodynamic was determined by echocardiography, tetrapolar rheography.
After  six-month of treatment both E and L improved NYHA class, 6-minute walk distance, and quality-of-life scores (all p<0,05 from baseline), and there were no differences between the 2 treatments.
Increased glomerular filtration rate (GFR) and effective renal blood flow (ERBF) were observed in the course of treatment with E (GFR – 68+1 ml/min before treatment vs. 71+2 ml/min after treatment, p<0,05; ERBF – 621+16 ml/min vs. 781+12 ml/min, p<0,01, respectively). E increased renal sodium and water excretion accordingly by 32.1-74.3% and 21.3-64.6%. The renal vascular resistance decreased by 16.2%. The change in the L group was significantly greater than in the E group, especially about concerning ERBF and renal vascular resistance.
Our results show, that beneficial effects of E or L on renal function and hemodynamic parameters were due to direct renal action of the drugs (selective renal vasodilation due to angiotensin II inhibition). Improved natriuresis and diuresis, decreased the rigidity of vessel’s wall, which was marked in CHF due to high sodium concentration, and also decreased the compression of vessels due to interstitial edema. Decreased renal vascular resistance. This course the potentiation of vasodilation. After six-month E and L treatment subjective complaints and objective symptoms of CHF decreased and tolerance to physical activity increased. Both drugs treatment reduced the functional class of CHF, which helps the rehabilitation of pts.

SUMMARY
 The pediatric asthma-school has been established for the purpose of appropriate medical education and enhance of access to health check information, as well as for the mastering in self-help and self-control methods for patients with asthma and their family members.
The efficacy of educational program in asthma-school was estimated by dynamic inspection of 105 patients with bronchial asthma. The age of children was 6-15. Individual programs for education of patients and their parents (in accordance to the parents’ approval), which included lessons with the patients and their parents according with the form and severity of the disease, were completed. The length of each lesson was 50-60 minute and the extent of each course was 10-12 lessons .The education course consisted of   academic lectures and conversations (Questions- answers) between patients, their parents and the Asthma-school members.
12 months after the end of the educational process, the connection of patient and his family members’ knowledge in management of Asthma and as a result, the positive dynamics of subjective and objective symptoms of the disease could be observed. There was a significant improvement in the condition of patients, as well as decline here the index of disease from 13,5 + 0,7 to 5,1+1,1 at the end of the educational round.
Thus, as a consequence of Asthma education there was developed not only the improvement of the basic knowledge of asthma among the patients and their relatives, but also, the progression in dynamics of the subjective and objective findings of the disease.

Summary.
The article represents the literary review of modern methods of treatment of the neurogenic dysfunction of the bladder in spinal cord injured patients and of methods of the bladder reinnervation.
The author concludes, that the most of current methods of treatment are symptomatic and less effective. The methods of bladder reinnervation: implantation of a sacral anterior root stimulator and operation of ileo-vesicopexy, represent the way of pathogenetic treatment and are much more effective and perspective.

Infectious Diseases, AIDS and Clinical Immunology Research Center. Research Institute of Pediatrics. Role of human herpesvirus-6 infection in children with fever and rash.

Summary. Human herpesvirus-6 (HHV-6) is one of the recently discovered herpesviruses. Primary HHV-6 infection is causative agent of roseola infantum, one of the diseases of childhood characterized by the development of fever and rash. The goal of our study was the evaluation of the role of primary HHV-6 infection in children with fever and rash. 32 children 6 months – 5 years of age with fever and generalized rash who have been tested to be negative on measles and rubella antibodies were enrolled in the study. Anti-HHV-6 IgM and IgG antibodies as well as HHV-6 DNA were detected in these groups by ELISA and PCR methods correspondingly. Primary HHV-6 infection has been found in 11 out of 32 patients  (34,4%) from the study group vs. none in the control group (p<0.001). Primary HHV-6 infection is associated with fever and rash in the early childhood and it has to be considered during the differential diagnosis of the diseases with rash.

It is known that bronchial asthma is the chronic inflammatory disease and that among other peripheral blood cells, erythrocytes have great importance to be investigated to understand the pathogenesis of this process.
   To estimate the efficacy of treatment before and after ZADITEN administration on the basis of structural and cytochemical data total 80 patients with atopic asthma were examined. Children with atopic asthma were divided into two age groups. We made morphological and cytochemical studies of peripheral blood erythrocytes in acute period of illness-before and after treatment. Our material we investigated with the use of method of direct erythro-cytometry and with the aid scanning electron microscopy. In the material the erythrocytes are presented differently before and after treatment. It includes: cell thickness, dry weight unity on surface, and concentration of dry substance unity on surface. The therapy with sympatomimetics, bronchospasmolitics, hormones do not restore the whole structural and cytochemical changes of erythrocytes. Therefore, we observed after ZADITEN administration in both groups significant restoration of erythrocyte membranes. According to this data, we can judge about the efficiency of the treatment, the severity of disease and the ability of the prognoses of the disease.

Summary. Surgical treatment remains as method of choice for scary stenosis of laryngotracheal area. 51(85%) out of 60 patients with laryngotracheal scary stenosis have been operated  using several types of plastic-reconstructive surgery  in Georgina center of thoracic surgery science and methodologies at Thoracoabdominal clinic in Tbilisi Georgia. Types of surgery implemented. Circular tracheal resection have been performed in 44 cases and several types of laryngotracheal plastics have been performed in remained 7 cases. Preoperational tracheal dilation with rigid bronchoscope have been utilized in 46 patients with total of 76 procedures performed. Perioperational antibacterial prophylaxis have been utilized in 37 cases (group 1). 14 patients (group 2) have been operated without any of perioperational antibacterial therapy. In 3 (8,14%) cases from group 1 have been observed several postoperational  complications, rather then in group 2  10 (71,1%)  cases have been presented with postoperational complications. Overall mortality rate was 3,9% and all of them from group 2 – patients operated without antibacterial prophylaxis.

Resume. Psychic disorders developed in the period of gerontogenesis represent an issue of great practical significance in psychiatry.
In our opinion, the significance of the present research lies in the fact, that the study of the regularity of the course of schizophrenia for the whole period of its development will most likely give us an opportunity to define the near and distant prognoses of the  desease with greater precision. On the basis of the results of the research, we shall be capable of objectively solving all practically important problems related to the treatment of the desease and its rehabilitation. Moreover, it is possible to make preliminary conclusions which specify that continuous paranoidal schizophrenia started at the age of  18-25 is characterised with a much worse distant prognosis compared to the same desease  started at a later age /25-32 /.

A b s t r a c t
The paradigm of objective estimation of frequency discrimination facilities in cochlear im­plant users (CI) has been elaborated. The idea of the procedure basis on the frequency spe­cific charac­teristics of the slow cortical potential (SCP). The tonal stimuli of two different fre­quen­cies are presented to the listener through the loudspeaker at fixed inter-stimulus intervals. Two ref­erent trials and a single test trial have been conducted. In the referent trials two SCPs are aver­aged in two separate computer channels to the consecutive presentations of two sets of tone bursts of two different frequencies. In the test trial the tone bursts of both frequencies are mixed and ap­plied under the same stimulation run alternatively, i.e. one after another. As in the referent tri­als, in the test trial the SCPs to the tone bursts of each frequency are averaged in two separate com­puter chan­nels. Two SCPs of the test trial are then compared with two re­spective SCPs of the referent trials. In the case of discrimination of the presented frequencies by the listener, e.g. by the CI recipient, two differ­ent popu­lations are activated in the SCP gen­erating system in the test trial. The actual intervals between activation of each population, destined for each frequency, are correspondingly pro­longed that is followed by the amplitude increments. The amplitudes of the test SCPs exceed therefore those of the respec­tive referent SCPs. In the case of the absence of frequency dis­crimination, conversely, the same neural population is triggered under both mono-tonal and bi-tonal stimu­lation that is manifested in the amplitude equalities of the respective test and referent SCPs.
Generally, the adopted procedure is considered beneficial for precise estimation of fre­quency discrimination aptitudes in individual CI users. The objective assess­ment of dis­crimi­nation facilities of applied sound messages could particularly useful in prelin­gual pa­tients, es­pecially in young children.

S u m m a r y 
Eye-socket phlegmon is devoloping from the odontogene inflammatory processes – near-lying anatomic formations of the eye-socket. Eye-socket phlegmon is characterized by serious clinical course, frequent suppurative complications in retrobulbar area, optic nerve atrophy what often brings to reduction of eyesignt keenness and total blindness.
The Maxillary –Fecial Surgical Department of the Tbilisi No. 4 Clinical Hospital within the last 6 monthes has been treating 3 patients with diagnosis: Eye-socket phlegmon in retro bulbar area, We carried out surgical dissection of eye-socket through upper, lower and lateral accesses. Was carried out Highmorotomy, dissection of retro bulbar athmoid area as well as complex medicinal treatment.
Two (2) patients lost eyesight; one (1) patient  fully recovered. Conclusion: Our observations permit us to express an opinion that timely diagnosing and surgical-complex treatment of eye-socket phlegmon and retro bulbar area makes full recovery possible.

 Due to the diversity of opinion in the literature describing temporal area, layered preparation of these areas is carried out on the adult and infant corpse material. It is confirmed that infectious processes can easily spread and generalize in soft tissues of infant temporal areas. In adults, seven connective tissue layers are discovered, which are sharply delineated, differently isolated, and connected to the neighboring areas. The consideration of their existence and distinctiveness is necessary for the prognosis, diagnosis and treatment of the development and expansion of infectious  processes in the area.

Summary
The foundations of Georgian medical terminology  were first laid in 1918 when a medical faculty was opened at Tbilisi State University. The absence of Georgian-language medical terms was a great obstacle in the process of training local medical students. On the initiative and with the participation of Prof. A. Natishvili and Z. Maisuradze the first Georgian anatomical terminology was created serving further as  a basis for creating branch terminology in the science of medicine.
In 1994, reproductology (reproduction science) acquired in Georgia the status of an independent medical discipline under the class 14.00.50 giving start to collection of relevant information. The reproductology branch lacks reference information, whereas the demand for it is permanent growing.
The team of authors offers a  Georgian defining dictionary of reproductology terms on the one hand as well as the corresponding Russian and Latin equivalents on the other.
The concise Georgian defining dictionary of reproductive terms is another step forward in the classic university medical education taking its origins from foundation of the first Tbilisi University based medical department in 1918 as well as from the Zhordania Research Institute of Human Reproduction, which since 1958 has been engaged in the origination and development of the branch of reproductology in Georgia.


41. Khudjadze, Gogniashvili, Kevanishvili, Tracheal stenosis therapy by using resection-anastomosis method.

 Abstract
Tracheal stenosis is not quite frequent nozology , at the same time it is most complicated pathology in ORL. The matter is that two main methods of treatment such are dilatation and resection-anastomosis are connected with a great complicate and it requires highly skilled surgeries and anesthesiologists.
From ethiological factors must be mentioned trauma and scald although posttracheotomic stenosis must be separated. There are compensative and decompensative levels of stenosis, but mostly we deal with compensative dyspnoea what gives us oportunity to plan treatment tactics beforehand.
Our report is devoted  to a surgical treating of posttracheotomic stenosis by using resection-anastomosis method. 36 years man  was operated and observed after operation during five months.


42. Khvadagiani M.G. ORL Clinic of the Medical Academy, Tbilisi, Georgia. Therapy for poliposis of nose cavity.
The use  of rigid endoscopes for investigation of nasal sinuses and functional endonasal endoscopy for the treatment of poliposis are standard approach.
 As a first step we used antihistamin, antibacterial and antiiflamatury treatment for the patients with poliposis. After 3-5 weeks we reevaluated of patients and than according to results consrvative treatment was continued or endonasal endoscopic microsurgery was sugested. From 77 patients treated by this in 21 cases consrvative treatment was effective, but 51 there was need for complex treatment modality (conservative + microsurgery). In 5 cases there was the need for repeated microsurgical intervention and only one patient underwent radical surgery.
 The complex therapy is reducing amount of patients whose need radical operation.

A total of 42 patients aged 30-75 years were studied. The paper provides evidence for the association of outcomes with type of reconstruction, continuity of surgery and volume of hemorrage.

We offer a new method of surgical treatment of lower prognathysm. Vertical osteotomy of the branches of lower jaw dawn to the lower third part is done, which is followed by horizontal osteotomy up to the first molar. The method is termed vertical-horizontal osteotomy of lower jaw. In the process of the upper prognation treatment, for the improvement of the osteotomical fragments by means of wire stitches or microplastic osteosynthesis is proposed.

On the average, at least one child in every classroom in the United States needs help for the disorder. ADHD often continues into adolescence  and  adulthood, and can cause a lifetime of frustrated  dreams and emotional pain.
But there is help . . . and hope. In the last decade, scientists have learned much about  the course of the disorder   and are now able to identify and treat children, adolescents and adults who have it. Patients with chronic heart failure (II, III NYHA) were found to have significant reduction of Fe3+ transferin with Fe2+ accumulation and the enhancement of lipid peroxydation.
This is significant reduction of NO concentrations, possibly as a result of low expression by endothelial cells of NO and peroxynitrite transformation of the latter, or, alternatively, HbNO and FeSNO complex formation in the blood of patients with chronic heart failure

Summary
The mechanism of intraradical  use of dentin adhesive systems and etching acids of hard tissues is unknown today. Therefore our research was aimed at studying the degree of their adhesion and penetration into tooth canal tissues with the help of scanning electronic microscope.
3 various adhesives AQ Bond (Morita), Etch & Prime (Degussa) and Prime & Bond NT (Dentsply) were used in the process of the research. 16% EDTA and 35% H3PO4 were selected as etching components. Human extracted teeth were used as objects of the research. The sections of teeth were studied with scanning electronic microscope after their treatment with the above-mentioned systems.
Adhesive system Etch & Prime 3.0 (Degussa) together with 16% EDTA demonstrated the best qualities. Dissolution of smear layer on the canal wall after the operation of EDTA and the exposition of dentin apertures stimulated penetration of the adhesive into deep layers of dentins and formation of wide hybrid zones.

Summary
It has been established that kidney cells of a mature white rat include inhibiting albuminous factor for epiteliocytes` reproduction. It's shown that the effect of the mentioned factor is performed at the level of genes` original activity regulation. Rat`s kidney albuminous factor reveals an inhibiting effect on the nuclear transcript activity isolated from the human cells of Renal cell carcinoma (light cellular variant) as well. The factor`s inhibiting effect has not been revealed at the area far from a tumor center that may be conditioned by the limitation of target genes` exscess to mentioned cells.

In the work there are submitted children's groups of increased risk, which are formed at children in the preschool and school period under influence of negative biological and socio-psychological factors, mainly with sub clinical signs and with demonstration in pubertal period in the form of delinquency. There are developed early concrete and differential preventive actions of social disadaptation.

Summary
For last years the special urgency is got with a problem of deviant behavior among children and teenagers. Among various forms of dissocial frustration, basically, it is submitted delinquent behavior. Delinquency more and more becomes complicated with heavy aggressive, unethical and immoral displays, early alcoholization, glue sniffing, "rejuvenescence" of an addiction, and that is especially important, frequent illegal behavior and criminal acts. Abovementioned manifestations are mainly shown at addictive variant of delinquency. At teenage age it is not actual generated addiction, but the malicious use of narcotics and toxic substances, when the expressed individual physical dependence is not marked and disease is not developed yet.
The mane purpose of research was to study addictive variant of delinquency, developed under influence of negative microsocium and chronically psycho traumatic factors. Among the investigated 50 teenagers, factors of negative microsocium are revealed in 41 %, and chronically psycho traumatic factors - 59 %. Proceeding from the analysis of a material, types disharmonic families were showed in the following sequence: incomplete family - 53 %, rigid pseudosolidar family - 21 %, breaking up family - 9 %, the broken family - 3 %.
Among the types of upbringing hipopatronage have been established in 53 %, the hyper patronage - 12 %, inconsistent type of upbringing - 15 %, changing of upbringing methods in connection with dynamics of home-life - 15 %, upbringing as family "idol" - 6 %. Among family conflicts on the forefront there came intensive family conflicts of-76 %, then incidental - 16 % and easy family conflicts - 8 % act.
 Character accentuation, psychopath, pathological pubertal crisis, asinqronous rate of maturing, mainly an acceleration, can be ranked as factors promoting development of delinquency and, in particular, its addictive variant.
Illegal behavior at addictive variant of delinquency there is established in 35 %, and criminal acts - in 6 %. 
In addictive variant, prevail teenagers with underdeveloped moral - ethical standards and absence of correct social motivation, though it is necessary to note, that social adaptation is not broken for long time.
The orientation of addictive teenagers behavior is determined by aspiration to get in "narcomanic" campaign and to "associate" in it. They exaggeratedly imitate the leader of antisocial group, participate in getting alcoholic drinks and toxic means and, in general, in the organization "addictive climate ". The certain part of teenagers shows ability to analyze critically the behavior and frequently keeps an opportunity of prognostic estimation of addictive behavior consequences

Summary
The influence of prevention by immunomodulator and sedative premedication on the genes expression in cells of lung and liver tissues of experimental animals (white rat) at inhalative anesthesia with halotane was studied. It was established that inhalative anesthesia via halotane inhibits the process of transcription only in nuclei of lung cells. The inhibition effect stipulated by anesthesia is removed after premedication with diazepam. The analogous effect was observed also after prevention via plaferone. At thf same time the inhibition of RNA- synthesis takes place in cells of both tissues by simultaneous application of abovementioned drugs.

The surgery consists of the following stages :  1) cutting the liver of the general blob circulation by means of a roundabout shunting without using the one - way  pump;   2) conservation of the liver;   3) spontaneous intervention on the liver parenchyma;   4) revascularization.

Summary
Aim:  The goal of our research was to investigate the changes of oxidative–antioxidative systems occurred during the experimental traumatic shock.
Methods: Experiment was held on 30 white rats weighing 200gr. Traumatic shock was induced by the injury of soft tissues according to the Cannon method. The decreasing of the arterial blood pressure was concomitant of the shock development. 60 mm of the mercury line was the threshold of the shock development. 15 minutes later (from the  shock development) the first group animals were injected by the saline 0,3ml intraperitoneum.  The animals of the second group were injected by Plaferon LB 0,6 mg on  0,3 ml the saline. 30 minutes later from injections, rats were killed. The blood was taken from the jugular vein and was studied by biochemical and electron spin resonance (ESR) spectrometry technique on radiospectrometer RE-1307.
Results: The study revealed that traumatic shock is characterized by activation of lipid peroxidation, hyperproduction of free radicals and inhibition of endogenic antioxidant system. Plaferon LB significantly decreases processes of lipid peroxidation and improved antioxidative capacity of blood.

In 1997-2002 years 85 patients with thoracal injuries  were admitted our hospital.Surgical operations were performed in 52 cases-pneumoraphy-7, cardioraphy-3, ligation of aa. Intercostals et mamaria inferna-4, pericardioraphy-1, lobectomy-1, diaphragmoraphy-7. In 25 cases invasion was limited as surgical cleansing of wound and drainage of pleural cavity.
We suppose that surgical invasion must be maximally limited- with strictly according to the indications.

   The explosion of electrical  station of 2001 was important for same reasons: the explosion itself is a big stress because it’s power and strength  is egual to explosion barotrauma and it causes the mental desorders of craniocerebral trauma;

 The impact of the substance after explosion is as a toxical factor:

The people settled their next to the power station(The technical and hygiene living condition were rejected)

They had before Post-Traumatic Stress Disorder(PTSD) and they have been living in that teritory under the fear.

There were following traumatic stress factors:

     1) Psychophysiological stressors-strange and terrifying noise,screams of agony,scattering of dust and the strange substances and pain due to injuries.

2)      Information stressors(what is going on? how can I escape? where are my parents or children)

3)      Emotional stressors(threat of death. the fear for one’s self and for parents, vital feeling, hopeless)

4)      Social stressors (the demage of house ) and another material harm

The results showd that after exlosion 80% of those inhibitants have PTSD, 10%-Depression,

8%- Phobic syndrome.Almost everyone have the signs of barotraume.

As a result of this ecological equilibrium was disturbed and traditional living conditions were violated.

    The traumatization of these people are in progress for 15 years and the top level of it was 22 Desember,when some stress disorders were cumulated which worsening  their health state.

Contributing to the existing economic problems consequences of the above mentioned are: desocialization, desadabtation, PTSD, , anxiety, behavior disorders, chronic personology disorders

     At present the traumatic stress factor still remains, because they live in the same area.

The study reveald that mental state in these groups is critical and needs urgent management and support, more active complex help in medical and social field.

SUMMARY
Aim of the work is to study the possibilities of use of plasmatic stream with the medical aim. The work of experimental and clinical character. By means of experiments carried out on the rabbits the influence of plasmatic stream on microorganism, as well its influence on the processes of reparative regeneration. The analysis of the results obtained have shown that the plasmatic radiation has a powerful bactericidal affect, it causes the stimulation of reparative regeneration in soft tissues of the organism.
The use of plasmatic stream in a complex treatment of above indicated diseases considerably speeds up the time of treatment and has a grate economical effect.

Summary
This study investigated the reactions of neutrophil in immature newborns during neonate sepsis. The amount of neutrophilic granulocytes
(NG) with active phagocytosis (absorbtion and killing of bacterial antigen) – “classical’’ type of reactivity were studied. The “paradoxicalal” reaction – necrosis (granulolysis, incomplete phagacytosis) also is demon-strated in peripheral blood specimens.
The results suggest that immature induces the different functional activity of NG in newborns

Tuberculous meningitis remains one of the severest and most serious diseases among various forms of tuberculosis up today.
The prognosis for the disease depends on the timely diagnosing and conducting adequate antituberculous treatment. The relatively low percentage of the cases of tuberculous meningitis and relatively high percentage of tuberculous meningoencephalitis testify to the belated diagnosing. The bulk of the deceased patients are the ones with belated diagnoses.
In the division of tuberculous meningitis at the National Center of Tuberculous and Lung Diseases 236 patients were admitted from 1995 to 2002. Among them 208 patients were diagnosed as tuberculous meningitis and tuberculous meningoencephalitis. In 28 cases the diagnoses of tuberculous meningitis were not confirmed. They were diagnosed later as virus meningitis, disturbances of blood circulation in brain, volume processes in brain and the patients were transferred to the adequate clinics.
From 208 patients 155 ones were cured, 11 ones were discharged in unchanged states, 7 ones – with improved states, in 2 cases we had aggravation in the form of central nervous system tuberculosis, 35 patients died, among them 28 ones in the very first days of their admission, and 7 ones due to ineffective treatments.

Summary
Development of the drugs with the effects against ischemia  of the kidney is the great challenge. Plaferon  was developed at the Institute of Biotechnology (Tbilisi, Georgia; director – V. Bakhutashvili), that was successfully used in clinical practice during last decade. Plaferon  is produced from the amnion of the human placenta, undergoes special treatment to avoid contamination by hepatitis B and C viruses as well as by HIV.
The aim of our study was to stress the effect of Plaferon-  in experimental setting on renal function, structure, hemodynamics and metabolism in case of ischemia for 60 min and 90 min. Comparison has been made between the local hypothermia and pharmacoprotection by Plaferon LB as a protector against renal ischemia.
The experiments were carried out on 100 wistar rats (weigh 150-200 g) and 72 mongrel dogs (weight 15-24 kg). Our studies revealed that Plaferon  is the potent anti-ischemic and protective drug against renal ischemia. The survival of animals was high – the evidence of positive activity of Plaferon

S U M M A R Y
The aim of the research was to study whether ACE-inhibitors may be used in patients with chronic heart failure (CHF) of high functional classes (FC) and arterial hypotension. The findings support the fact that arterial hypotension is of relative significance in patients with CHF when the max effective doses of  ACE-inhibitors are chosen. The development of antihypertensive responces to ACE-inhibitors in patients with baseline hypotension without significant clinical manifestations and no less then 85 mm Hg SBP reduction produces no negetive effect on central hemodynamic parameters and exercise tolerance.

Complex Treatment of  Long Bones Segmental   and Soft   Tissue Septical   Defects of Upper Limb.
 Summary.
The study cohort comprised 26 patients, who were treated in O.Gudushauri Centre of Orthopedic and Traumatology  and Sepsis centre of Georgia  from 1986 to 2002 years, with long bones segmental and soft tissues  septical defects of upper limb. Average age  40 years . From 26 patients, .23 was male and 3 female. 59% of patients were with nonunion fractures,  after high  energy  open trauma. 41 % of   patients had   complications  after   combine   fractures by  non stabile osteosinthesis. All  patients    before  our treatment, spend   in other clinics from 1 month to 1 year. Some of them  had  from  1 to 3 operations without result. In 1981-86 years in 68 %   cases grew up staphylococci microbes, the gram-negative infection was  32%.  In present we had another information from microbiology laboratory of sepsis centre, staphylococci flora grew in 64%, gram-negative microbes - in 36 %, effectiveness of popular antibiotic ciprofloxsacilline - in 63 %, penicciline – in 16%,amoxicilline – in 12%.  Before operations we used antibiotics after test with pathogenic microbes from the wound. From 152 cases of grafting long bones septical defects with cortical cancellous and spongioza auto grafts upper limb was in 26 cases (17%).  In 6 cases there was skin and soft tissue defects of arm and forearm. To covered soft tissue defects in  2 cases  with  big area of skin defect, we use free vascularized abdominal flap, in other cases we use free flap of skin. In  5  cases  bone grafting used before skin plastic. The positive result  after our treatment  we watch in 24 cases, in 1 patient after gunshot trauma of arm the bone was nonunion, without infection, in 1 case was recidive of osteomielit.
